Inpatient optimization of Guideline-Directed Medical Therapy for Heart Failure with Reduced Ejection Fraction (EF ≤40%).
1. Stabilize & Stratify (Admission Day)
Before initiating GDMT, confirm the following:
- HFrEF confirmed (EF ≤40%) — identify precipitating factors
- Hemodynamic profile stratified: warm/wet · cold/wet · warm/dry · cold/dry
- Symptomatic NYHA II–IV HFrEF or HFimpEF that still qualifies for GDMT
- Decongestion achieved or actively underway with IV loop diuretics
- Baseline labs documented: SCr, eGFR, K⁺, BNP/NT-proBNP, glucose
2. Gate to GDMT Initiation
All criteria should be met before starting any GDMT pillar:
| Criteria | Target |
|---|---|
| SBP | ≥100 mmHg for ≥6 h without IV vasodilator |
| Inotropes | None in prior 24 h |
| Diuresis | Transitioning toward oral diuretics; stable weight and urine output |
| Oxygenation | Adequate; no symptomatic hypotension or shock |
3. Pillar 1 — RAAS Inhibition (Prefer ARNI)
Sacubitril/Valsartan (Entresto) — First Choice
| SBP | Starting Dose |
|---|---|
| 100–120 mmHg | 24/26 mg BID |
| >120 mmHg | 49/51 mg BID |
- ACEi washout: Enforce 36-hour washout before switching from any ACEi to ARNI (angioedema risk)
- Renal: eGFR >45 preferred; acceptable down to ~30 with close monitoring
- Creatinine: Tolerate up to ~30% SCr rise before dose adjustment
- Recheck BMP within 24 h of initiation
- Up-titrate every 1–2 days as tolerated; advance to next GDMT pillar once moderate dose achieved
If SBP Persistently <100 mmHg
Start low-dose ACEi or ARB as bridge:
- Lisinopril 5 mg daily or Valsartan 20 mg BID
- Plan to upgrade to ARNI once hemodynamics allow
PARADIGM-HF: Sacubitril/valsartan reduced CV death/HF hospitalization by 20% vs enalapril
4. Pillar 2 — Beta-Blocker
Initiate once ARNI/ACEi/ARB is on board and patient is warm, non-hypotensive, not on inotropes.
Starting Dose
| Condition | Agent & Dose |
|---|---|
| SBP 90–120 mmHg or weight <85 kg | Carvedilol 3.125 mg BID |
| SBP >120 mmHg or weight ≥85 kg | Carvedilol 6.25 mg BID |
| Intolerant to carvedilol | Metoprolol succinate 12.5–25 mg daily |
| Alternative | Bisoprolol 1.25 mg daily |
- Continue pre-admission beta-blocker unless: shock, severe pulmonary edema, or true intolerance
- Up-titrate every 1–2 days as tolerated
- Relative contraindication: HR <50 bpm
- If symptomatic hypotension develops, reduce beta-blocker before reducing other pillars
- Consider switching carvedilol → metoprolol succinate if hypotension is problematic
MERIT-HF, COPERNICUS, CIBIS-II: Three evidence-based agents only — carvedilol, metoprolol succinate, bisoprolol
5. Pillar 3 — Mineralocorticoid Receptor Antagonist (MRA)
Initiate after RAAS inhibitor + beta-blocker are established.
Eligibility
| Parameter | Threshold |
|---|---|
| SCr (men) | ≤2.5 mg/dL |
| SCr (women) | ≤2.0 mg/dL |
| eGFR | >30 mL/min/1.73 m² |
| Potassium | <5.0 mEq/L |
Starting Dose
- Spironolactone 12.5 mg daily (titrate weekly toward 25–50 mg)
- Eplerenone 25 mg daily (preferred in men with gynecomastia risk)
Monitoring
- Recheck BMP within 7 days of initiation
- Repeat at 1–2 months
- MRAs have minimal BP effect — safe in borderline SBP
- Consider potassium binder (patiromer) or SGLT2i co-initiation to mitigate hyperkalemia
RALES (spironolactone) and EMPHASIS-HF (eplerenone): Both demonstrated significant mortality and hospitalization reduction in HFrEF
6. Pillar 4 — SGLT2 Inhibitor
Initiate before discharge (can precede or follow MRA).
Agents & Dosing
| Agent | Dose | eGFR Threshold |
|---|---|---|
| Dapagliflozin | 10 mg daily | ≥30 mL/min/1.73 m² |
| Empagliflozin | 10 mg daily | ≥20 mL/min/1.73 m² |
What to Expect
- Modest BP drop: ~1–4 mmHg
- Initial eGFR dip: ~4–6 mL/min/1.73 m² — typically benign and reversible
- Adjust background insulin/sulfonylureas to avoid hypoglycemia
- Reinforce sick-day rules and DKA warning signs
Contraindications
- Type 1 diabetes
- eGFR <30 (dapagliflozin) or <20 (empagliflozin)
DAPA-HF and EMPEROR-Reduced: Both showed significant reduction in HF hospitalization and CV death, independent of diabetes status
7. Loop Diuretic Transition
- Trial oral diuretics ≥24 h before discharge with liberalized fluid intake to simulate home conditions
- Target the lowest effective dose that maintains euvolemia
- Consider torsemide if poor furosemide response or adherence issues (better bioavailability, once daily)
- Minimizing diuretic dose maximizes room for GDMT up-titration
8. Barrier Mitigation
Acute Kidney Injury
| Strategy | Action |
|---|---|
| First-line | Reduce/hold RAAS-I/ARNI; reduce diuretic to lowest effective dose; retrial RAAS once SCr improves |
| Second-line | Switch RAAS-I/ARNI to hydralazine/isosorbide dinitrate only after multiple failed attempts |
Hyperkalemia
| Strategy | Action |
|---|---|
| First-line | Stop K⁺ supplements; add potassium binder (patiromer); low-K⁺ diet; add SGLT2i; retrial ARNI |
| Second-line | Reduce/hold RAAS-I/ARNI or MRA one at a time; hydralazine/nitrate only after multiple failed attempts |
Symptomatic Hypotension
| Strategy | Action |
|---|---|
| First-line | Remove non-GDMT BP agents; stagger GDMT dosing (morning/evening); reduce diuretic burden; reduce GDMT dose based on symptoms, not BP numbers alone |
| Second-line | Prioritize beta-blocker; switch carvedilol → metoprolol succinate; reduce RAAS-I/ARNI dose; reduce SGLT2i and retrial at regular dose later |
Adherence / Cost
| Strategy | Action |
|---|---|
| First-line | Pill organizers, smartphone apps, once-daily formulations; submit prior auths early in hospitalization |
| Second-line | Post-discharge telehealth; refer to HF medication titration clinic; reassess ability to pay; institution-specific cost analysis |
9. Pre-Discharge Checklist
- All four GDMT pillars active — or contraindications documented
- Oral diuretic plan confirmed (agent, dose, instructions)
- Iron deficiency addressed if present (ferritin <100 or 100–300 with low TSAT)
- Labs ordered: BMP, glucose, BNP/NT-proBNP
- Repeat labs scheduled within 1–2 weeks post-discharge
- Follow-up: telehealth or in-person within 7 days; HF specialty clinic referral
- Cardiac rehab referral placed
- Patient education completed: medication purpose, symptom diary, daily weights/BP, sodium/fluid guidance, sick-day rules, smoking cessation
10. Post-Discharge Trajectory
| Timeframe | Action |
|---|---|
| 7 days | Telehealth or clinic follow-up; check vitals, weight, labs |
| 1–2 weeks | BMP recheck (K⁺, SCr, eGFR); adjust GDMT as needed |
| 1–2 months | BNP/NT-proBNP for response stratification |
| Every 1–2 weeks (ongoing) | GDMT dose escalation via clinic or telehealth until guideline-max doses reached |
| ~3 months post-GDMT optimization | Repeat echocardiogram to reassess EF and evaluate device candidacy (ICD/CRT) if EF remains ≤35% |
